Give Feedback

This provides an easy way to share suggestions, report issues, or request new features for the ADAPT tool. Using the "Give Feedback" button located in the left-hand navigation menu, users are redirected to GitHub, where they can create and submit feedback. Please note that a GitHub account is required to submit feedback.

To provide feedback, follow the steps below:

Select Give Feedback button

Select the Give Feedback button in the left-hand navigation menu.

Log in to GitHub

You will be redirected to GitHub to log in.

A GitHub account is required to submit feedback. If you already have an account but are not signed in, log in to your account to proceed.

Select "New Issue"

Once logged in, select the "New issue" button on the GitHub page to create a new feedback entry.

Choose the Type of Issue

Select the appropriate issue type from the following options:

  • Bug Report: Report technical issues or errors in the ADAPT tool.

  • Feature Request: Suggest new features or improvements.

  • Blank Issue: Provide general feedback or comments if the predefined categories don’t apply.

Fill Out the Feedback Form

After selecting an issue type, you will be directed to a form. Provide the necessary detail

Submit Your Feedback

Once the form is completed, select the "Create" button to submit your feedback.

Contact Assigned TA Provider

Contact your assigned Technical Assistance (TA) provider for personalized support with the ADAPT tool. Your TA provider can assist with troubleshooting, provide guidance, and answer questions specific to your needs. Contact information for your TA provider is typically provided during onboarding.
